As the parent/guardian of the above named child, I give him/her permission to participate in the activities of Catalyst Youth Group at Vision Conference 2025, Great Falls MT, October 30-November 1 2025. I release Christian City Church, New City Church, and their representatives from any liability in the event of an accident enroute, during, or returning from the conference. I also authorize them to perform any emergency medical attention that may be required during my child’s time with them.
