Consent and Release
I, the undersigned parent/legal guardian of the above-named youth, hereby give permission for my child to participate in the recording of the above-mentioned song/project.
I understand and agree that:
The recording may be used, reproduced, distributed, or broadcast publicly (including online streaming platforms, radio, and social media) as part of this project.
The youth participant will not receive financial compensation unless otherwise agreed in writing.
The youth participant’s voice and/or image may appear in promotional materials, videos, or photographs connected with the project.
The organization/studio has full rights to edit, mix, and distribute the final recording.
Participation is voluntary, and consent may be withdrawn before the recording session.
